29 CFR 1926.501(b)(1): Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems or personal fall arrest systems: a) This violation occurred on or about January 14, 2015, when at least two (2) employees, who were removing air conditioning units from the roof a single-story commercial building which had unprotected edges approximately 16-feet above the lower level, were exposed to the hazard of falling to the lower level.

29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes: a) This violation occurred on or about January 14, 2015, and at times prior thereto, when at least two (2) employees, who were removing air conditioning units from the roof of a single-story commercial building, were not protected from falling through skylights which were approximately 16-feet above the lower level.
