OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck by and/or crushed-by hazards.  This violation occurred on or about January 21, 2015,  in the extrusion area, the pendant controls to the CM Lodestar 2-ton overhead hoist being used to move up to 750lb parts did not have legible function labels exposing employees to struck by hazards.

29 CFR 1910.212(a)(1):  Machine guarding was not provided to protect operator(s) and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks;                                                                                                                   a) This violation occurred on January 21, 2015, and at times prior thereto, in the maintenance area, employees operated Clausing drill presses serial numbers 120220, model # 1751 with no guard over rotating parts exposing employees to amputation hazards.        b) This violation occurred on January 21, 2015, and at times prior thereto, in the shipping area, employees operated Dayton drill press model # 3Z919F serial number not legible with no guard over rotating parts exposing employees to amputation hazards.      c) This violation occurred on January 21, 2015, and at times prior thereto, in the maintenance area; employees were using a Makita saw blade size 12 inches with no guard exposing employees to amputation hazards.

29 CFR 1910.253(b)(4)(iii):  Oxygen cylinders in storage were not separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet (6.1 m) or by a non-combustible barrier at least 5 feet (1.5 m) high having a fire-resistance rating of at least one-half hour:                                                                                                               This violation occurred on January 21, 2015, and at times prior thereto, in the maintenance area, the employer stored oxygen cylinders next to flammable acetylene cylinders and other mixture cylinders exposing employees to fire hazards.
