29 CFR 1904.39(a)(2): 29 CFR 1904.39(a)(2): The employer did not report the in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours. a) Assembly department press P 038. An employee had three fingers fractured and the tip of one finger amputated while operating a punch press. The employer failed to notify the Occupational Safety and Health Administration of the amputation within the specified twenty-four hour period.
