29 CFR 1910.37(a)(3): Exit route(s) were not kept free and unobstructed:    a- The aisle along the back wall in Hot-Melt Building (C-5), serving as the access route to either rear emergency exit door, was obstructed by drum(s), pail(s), and box(es) placed within the aisle way, as well as by braided steel hose(s) hung from overhead hoist hook.  These materials / equipment reduced the exit route width to less than the minimum 28".    The condition(s) existed on 1/21/2015 and at times prior thereto.

29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered:    a-  Revised procedures for tanker truck loading were instituted in 2014 that provided for by-passing the hard-piped in-line strainer box of fully-enclosed design; in favor of pouring / straining the modified molten asphalt product through a mobile open-top strainer box using manual hose connections to the fixed pipe feeding the pump.  The revised procedure exposes Hot Mix batch operator(s) to the hazard of being splashed / sprayed by molten asphalt at or about 380 degrees Fahrenheit.  Effective barrier protection for the body, of non-synthetic material, was not furnished & required by the employer.     The above condition(s) / exposure(s) occurred on 1/21/2015 and at times prior thereto.

The employer had not performed a hazard assessment to determine needed personal protective equipment (PPE), such as effective barrier protection for the body, of non-synthetic material, as well as face protection.

A patch of damaged, missing concrete with outward-extending crack(s), near the center of the aisle-way, posed a tipping hazard for operator(s) of solid tire forklift(s) carrying loads within this mechanical equipment aisle.
