Location: 130 West 70 Street NY, NY/ Basement to fourth Floor.  On or About: 01/21/2015.   a) Employees performing hoisting operations in the elevator shaft were not protected from falling by a fall protection system.  b) No railing provided on floor, preventing employees from entering the unprotected area.

29 CFR 1926.502(d)(15): Anchorage used for attachment for personal fall arrest equipment was not capable of supporting at least 5,000 pounds (22.2 kN) per employee attached nor was it designed, installed and used as follows: As part of a complete personal fall arrest system which maintains a safety factor of at least two; and under the supervision of a qualified person.   Location: 130 West 70 Street NY, NY/ Basement, 2nd and 3rd Floors.  On or About: 01/21/2015.  a) The anchorage point used by employees was not designed and installed under the supervision of a qualified person. Employees used 5/16 wire rope between two beams as a horizontal safety line.
