29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:      a) CARLE'S BRATWURST, INC. Bucyrus OH: On or about January 5, 2015, the employer failed to develop and implement an energy control program, complete with procedures and training to protect employees from injuries while they engaged in servicing activities. Employees were exposed to lacerations and amputations while performing tasks such as job setups and cleaning meat preparation machinery, including but not limited to:      PATTY-O-MATIC, INC. Patty making machine, SIR STEAK MACHINE, meat cuber, BIZERBA USA, Meat Slicer, HOBART Meat slicer, BIRO Band Saw, LELAND MIXER, and BUTCHER BOY meat grinder.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by, ingoing nip points, rotating parts, flying chips and sparks on the following machines:    a) CARLE'S BRATWURST, INC. Bucyrus Ohio; On or about January 5, 2015, the employer did not assure employees were protected from nip point hazards while operating meat preparation machinery to include: "PATTY-O-MATIC, INC.", Patty making machine, Serial number 153, located in the meat preparation area.

29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employees from having any part of their body in the danger zone during operating cycle(s):   a) CARLE'S BRATWURST, INC. Bucyrus Ohio; Meat Department. On or about January 5, 2015, the employer did not assure employees were protected while operating the "SIR STEAK MACHINE" to cube meat. During operation the guard for the point of operation could be lifted and the interlock would not stop the blades from turning, posing a caught by and amputation hazard.
