29 CFR 1910.119(d)(3)(ii): The employer did not document that equipment complies with recognized and generally accepted good engineering practices.    The employer does not ensure that equipment in the process complies with recognized and generally accepted good engineering practices.        a. The violation occurred in the ammonia refrigeration energy center (or engine room) on or about January 15, 2015 and at times prior thereto where the employer failed to ensure visual and audible alarms were installed outside the engine room doors (South and East side of the engine room) entrance in accordance with ANSI/ASHRAE-15 (American Society of Heating Refrigeration Air Conditioning and Engineering) and ANSI/IIAR-2 (International Institute Ammonia Refrigeration). This condition exposed employees to inhalation, fire, and explosion hazards from releases of anhydrous ammonia.     b. The violation occurred in the ammonia refrigeration energy center (or engine room) on or about January 15, 2015 and at times prior thereto where the employer failed to ensure ammonia detectors were set at a value not greater than TLV-TWA (Threshold limit value-time weighed average) in accordance with ANSI/ASHRAE-15 (American Society of Heating Refrigeration Air Conditioning and Engineering). This condition exposed employees to inhalation, fire, and explosion hazards from releases of anhydrous ammonia.       Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date the employer must submit documentation showing that it is in compliance with the standard, including describing the steps it is taking to ensure that equipment in the process complies with recognized and generally accepted good engineering practices.
