29 CFR 1926.403(b)(1)(iii): Safety of equipment was not determined on the basis of electrical insulation.  a- Building A - Area directly behind offices -301 Riverside Drive, Millville NJ: On or about 01/21/15 a company employee was exposed to electrical hazards while utilizing a Makita 5007F electric circular saw which was powered with a cord whose outer covering were visibly damaged.    b- Building D - Area in the center of the building - 301 Riverside Drive, Millville NJ: On or about 01/21/15 a company employee was exposed to electrical hazards while utilizing a Makita 5007F electric circular saw which was powered with a cord whose outer covering were visibly damaged.    NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.1060(a)(1): The employer did not ensure that each employee was trained by a competent person which would enable each employee to recognize hazards related to ladders and stairs, and the procedures to follow to minimize those hazards.  a- Building A - Station 4 - Modular Building Section A2-219-01-A, 301 Riverside Drive, Millville NJ: On or about 01/21/15 a company employee was exposed to fall hazards when working from an A-framed ladders top step.  This employee had not received any ladder safety training from the company.    NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
