29 CFR 1910.178(l)(2)(ii): The employer did not ensure that each operator had successfully completed the training consisting of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace.    a)  On or about 02-22-15, the employer did not provide and/or instruct employees in adequate forklift operator safety training.  The employer's training did not include formal instruction as required by the standard.

29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been  taken out of service until restored to safe operating condition(s):  a)  On or about 01-22-15 at 3317 Behrens Parkway in Sheboygan, employer did not remove from service a Caterpillar 2EC25 forklift which had a broken reverse warning device.  An employee operated the forklift daily to unload palletized supplies from a semi-trailer and to stage the supplies inside the employee work area.
