On or about January 27, 2015, employees were exposed to trip hazards when the only way to get to the platform workstation for the employees working on the flex line for Lines #4 and #5 was to step over the structure supporting the overhead conveyor and the structure used to control tote storage location.

29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: On or about January 22, 2015, employees were exposed to struck-by and amputation hazards when a. The e-stop was used to stop the machine in order to re-thread the film if the operator did not splice new roll to the old roll outside of the machine. b. The e-stop was used to stop the machine in order to make adjustment to the heater bar, film alignment, or the part where the meat falls down.

The interlock on the film (packaging) machines permitted the machine to run with the door open. An employee received a partial amputation in 2014 when he was startled and placed his hand inside of the machine while it was running and the door was open.
