29 CFR 1910.23(e)(1): The stairway railing did not consist of top rail, intermediate rail, and posts, with a vertical height of 42 inches nominal from upper surface of top rail to floor, platform, runway, or ramp level the length of the stairway: Top of stairway, north wall of warehouse. On or about January 21, 2015, and at times prior thereto, employees were exposed to a fall hazard, as the upper portion of the stairway was not fully guarded with a standard railing, while they entered or exited the upper floor.

29 CFR 1910.253(b)(4)(iii): Oxygen cylinders in storage were not separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet (6.1 m) or by a noncombustible barrier at least 5 feet (1.5 m) high having a fire-resistance rating of at least one-half hour: Near the Welding area of warehouse. On or about January 21, 2015, and at times prior thereto, employees were exposed to burns from fire or explosion, as cylinders of oxygen and acetylene were stored on a cart directly adjacent to each other, as employees worked in the warehouse.
