A safe distance shall be maintained approximately three truck lengths from the truck ahead, and the truck shall be kept under control at all times. a-Kenco Logistic Services, LLC, 255 Blair Rd., Avenel, N.J. - The employer did not ensure that forktrucks on site are kept under control at all times. On/or about 01/16/2015 at the Kenco Logistic Services, LLC, Avenel, NJ site an employee operating a forklift truck did not keep control of the powered industrial truck, causing him to hit the back of another forklift truck and causing bodily injury to that operator. Condition noted on or about 1/23/2015.

On/or about 01/16/2015 at the Kenco Logistic Services, LLC, Avenel, NJ site an employee was operating a forklift truck at a speed which prevented him from stopping in a safe manner, causing him to hit the back of another forklift truck and causing bodily injury to that operator. Condition noted on or about 1/23/2015.
