29 CFR 1910.305(b)(1)(ii): Unused openings in cabinets, boxes, and fittings shall be effectively closed. a. On or about January 23, 2015, located in the fabrication shop behind the horizontal band saw, there was a 177 volt electrical panel box that was used in part to supply electrical power to the shop's overhead lights which had knockouts that were not effectively closed, thereby exposing the employees to a burn injury if an arc flash would occur. b. On or about January 23, 2015, located in the fabrication shop behind the horizontal band saw, there was a 120 volt electrical panel box that was used in part to supply electrical power to the shop's receptacle outlets which had knockouts that were not effectively closed, thereby exposing the employees to a burn injury if an arc flash would occur.

29 CFR 1910.179(g)(1)(iv): Where multiple conductor cable is used with a suspended pushbutton station, the station must be supported in some satisfactory manner that will protect the electrical conductors against strain. a. On or about January 23, 2015, located in the East Bay on the 10 ton overhead crane, the employer did not ensure that a pendant control box (suspended pushbutton station) had strain relief provided at the point where the multiple conductor cable connected to the pendant control box.
