29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous:  a) Jobsite - Employees were using various electrical powered equipment, including but not limited to a microwave and Ridgid circular saw without the use of a grounding prong.  Observed on or about 17 DEC 2014.   4 x 4 Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.404(f)(6), which was contained in OSHA inspection number 989466 citation number 1, item number 2a and was affirmed as a final order on 08 SEP 2014, with respect to a workplace located at 219 West George Street, Philadelphia, PA.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:   a) Jobsite - Employees constructing a 3rd floor framing project were exposed to a fall hazard of approximately thirty-five (35) feet without the use of conventional or alternative forms of fall protection.  Observed on or about 17 DEC 2014.  4 x 4 Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 989466 citation number 2, item number 1a and was affirmed as a final order on 08 SEP 2014, with respect to a workplace located at 219 West George Street, Philadelphia, PA.  4 x 4 Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 981496, citation number 1, item number 1a and was affirmed as a final order on 14 JULY 2014, with respect to a workplace located at 204 Laurel Street Philadelphia, PA

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  a) Jobsite -Employees were performing carpentry operations within 3 feet of an unprotected edge approximately thirty-five (35) feet above grade without being trained in fall protection.
