29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system: On January 23, 2015 and at times prior thereto; west side of building; employees were exposed to a potential fall of 19 ft. while working on an unguarded scaffold system (ladder-jack).

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: On January 23, 2015 and at times prior thereto; west-side; employees were using an extension ladder to access the roof area that was extended 6 inches above the platform exposing employees to a 20 foot fall to ground level.
