Office - On or about January 20, 2015, the employer failed to maintain OSHA 300 logs for 2014 and 2013 where injuries and illnesses had occurred at the establishment. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed: a) On January 27, 2015 and at times prior thereto; Boiler room location; the employer has developed lockout/tagout procedures but has not periodically inspected the procedures ensuring that they are followed and/or still apply to that specific equipment. The inspection must be documented with the inspections date, equipment, employee observed signature and the evaluators signature. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
