29 CFR 1910.9(b): Training. Standards in this part requiring training on hazards and related matters, such as standards requiring that employees receive training or that the employer train employees, provide training to employees, or institute or implement a training program, impose a separate compliance duty with respect to each employee covered by the requirement. The employer must train each affected employee in the manner required by the standard, and each failure to train an employee may be considered a separate violation. [FR 73 75583, Dec. 12, 2008] On or about January 22, 2015, employees exposed to a fall hazard as a result of an open sided floor that was not properly guarded by the host employer were not adequately trained to avoid the fall hazard.

29 CFR 1910.23(e)(5)(iv): The mounting of handrails shall be such that the completed structure is capable of withstanding a load of at least 200 pounds applied in any direction at any point on the rail. Location: Door 487; BOF pit On or about 22 January, 2015 employees were exposed to a fall from an open sided floor of over 20 feet into a pit that was not guarded by a standard railing that was capable of withstanding a load of at least 200 pounds, as parts of the railing were constructed from plastic safety tape.
