Location: Door 487; BOF pit  On or about 22 January, 2015 employees were exposed to a fall from an open sided floor of over 20 feet into a pit that was not guarded by a standard railing that was capable of withstanding a load of at least 200 pounds, as parts of the railing were constructed from plastic safety tape.
