When there is a change in work area conditions or degree of employee exposure or stress that may affect respirator effectiveness, the employer did not reevaluate the continued effectiveness of the respirator:  Worksite: When the employer, and his employees, voluntarily wore half-mask elastomeric respirators as protection against dusts and particulates the employer did not adequately evaluate the circumstances under which the respirators would be used.  Respirators were worn with the incorrect cartridges used as protection against dusts, and worn with excessive facial hair that would prevent a proper face-to-facepiece seal.
