OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees:     a) Roller Conveyor, west side of production area.  On or about January 22, 2015, employees were potentially exposed to hazards associated with the unintentional operation, activation, and movement of conveyor components from unguarded foot control pedals. The foot pedal that runs the roller conveyor was not protected to prevent unintentional operation.  Employees were exposed to hazards created by unintended operation of the conveyor which included but were not limited to fall hazards.

Employees who perform the servicing and maintenance work had not received authorized employee lockout/tagout training on topics that included but were not limited to magnitude of energy sources, sequence of lockout/tagout steps, machine energy isolation, machine energy control, lockout/tagout techniques and practices, procedures for shift and personnel changes, and lock/tag removal.

The servicing and maintenance work included, but was not limited to, setting up print jobs and cleaning debris from below the printing machines.  The equipment was not locked out when employees bodily entered the machinery to perform these tasks.

The loose wires were taped to the side of the machine with black tape.
