On or about January 22, 2015, employees were potentially exposed to hazards associated with the unintentional operation, activation, and movement of conveyor components from unguarded foot control pedals. The foot pedal that runs the roller conveyor was not protected to prevent unintentional operation.  Employees were exposed to hazards created by unintended operation of the conveyor which included but were not limited to fall hazards.
