29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.   On or about January 23, 2015, on the west side of the construction site, employees engaged in framing activities used pneumatic nail guns and were not wearing eye and face protection exposing employees to struck-by hazards and eye injuries.

On or about January 23, 2015, on the west side of the work site, employees engaged in framing activities utilized extension cords that were missing the ground pin exposing employees to electrical hazards.
