29 CFR 1910.23(a)(1): Open sided floor(s) or platform(s) 4 feet or more above the adjacent floor or ground level were not guarded by standard railings (or the equivalent as specified in 29 CFR 1910.23(e)(3)(i) through (v):    (a)  On or about January 20, 2015, and at times prior, the employer did not ensure that employees were protected from fall and struck-by hazards in the personnel lift shaft in the east elevator building in that the back side of the which was unguarded at each level of the elevator. 29 CFR 1910.272(j)(1): The employer did not develop and implement a written housekeeping program that established the frequency and the method(s) determined best to reduce accumulations of fugitive grain dust on ledges, floors, equipment, and other exposed surfaces:    (a)  On or about January 20, 2015, and at times prior, the employer did not ensure that employees were protected from grain dust explosions by removing fugitive grain dust from all exposed surfaces in the following locations:  1) The tunnel of the east building in the dust collection system that is no longer in use and runs adjacent to the conveyor belt.  2) The west building:      (i)   bin deck of the west building on the horizontal surfaces along of the distributor legs.      (ii)  in a rectangular box stored in the northwest area of the bin deck adjacent from the distributor legs.      (iii) on many pieces of angle iron attached at to the cement wall up the shaft of the personnel lift.    Abatement Note:  Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").
