29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used. Crossbraces were used as a means of access. The employer does not provide access ladders or other means for accessing scaffold platforms. This violation was observed on or about January 21, 2015, at a construction site when an employee was exposed to a 14 foot fall hazard while ascending and descending the frame of the scaffold that had no access ladder. Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that employees have proper access to the scaffold platform.

29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms before being released for use by employees other than erection/dismantling crews. The employer does not install guardrail systems along all open sides of scaffold platforms. This violation occurred on or about January 21, 2015, at a construction site when an employee was exposed to a 14 foot fall hazard while doing masonry work from the second level of a scaffold that lacked the required guardrails. Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that employees are protected from falling when working on scaffolds.
