29 CFR 1926.502(d)(15)(ii): Anchorages used for attachment of personal fall arrest equipment was not capable of supporting at least 5,000 pounds (22.2 kN) per employee attached, and not installed under the supervision of a qualified person. a) At the jobsite, on the Eastern shed roof in front of building 27, on or about 1/21/15: A worker was attached to a fall protection system where the anchorage was achieved by passing the lifeline over a lintel and attaching the snaphook to the lifeline. b) At the jobsite, on the Western shed roof in front of building 27, on or about 1/21/15: A worker was attached to a fall protection system where the anchorage was achieved by passing the lifeline over a lintel and tying it into a knot. Abatement certification must be submitted for this item.

PE Dwyer Construction Co. Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard CFR 1926.501(b)(3), which was contained in OSHA inspection number 191781, citation number 1, item number 1, and was affirmed as a final order on 10/9/12, with respect to a workplace located at 1055 East Genesee Street, Syracuse, NY.
