29 CFR 1926.501(b)(14): "Wall openings." Each employee working on, at, above, or near wall openings (including those with chutes attached) where the outside bottom edge of the wall opening is 6 feet (1.8 m) or more above lower levels and the inside bottom edge of the wall opening is less than 39 inches (1.0 m) above the walking/working surface, shall be protected from falling by the use of a guardrail system, a safety net system, or a personal fall arrest system.  a)  At the jobsite, on the 2nd floor, on or about 1/23/15:  Employees were working in close proximity to 5 separate floor-to-ceiling wall openings where guardrails were not installed, exposing workers to potential falls of 13 feet to ground.  b)  At the jobsite, on the 2nd floor, on or about 1/23/15:  Employees were working in close proximity to and transiting an area where a partition was installed with studs that were 20 inches on center, and where no guardrail was installed, exposing workers to potential falls of 13 feet to the lower level.

29 CFR 1926.501(c)(1): When employees were exposed to falling objects, the employer did not have each employee wear a hard hat and did not erect toeboards, screens, canopy structures or barricades to prevent objects from falling from higher levels:  a)  At the jobsite, on the 2nd floor mezzanine, on or about 1/23/15:  Employees were working and transiting through the lobby area where the mezzanine above was not fitted with toeboards, exposing employees to hazards from falling objects.
