29 CFR 1926.1053(b)(12): Ladders used where the employee or the ladder could contact exposed energized electrical equipment did not have nonconductive side rails: On or about 26 January 2015 employees at the jobsite were exposed to electrical shock hazards, when the ladder the employer provided for use by employees were using to access a fire escape to the roof of the structure was placed close to live electrical power lines and was not made of a non-conductive material. Employer did not ensure that a non-conductive ladder was used, nor that the ladder was placed a safe distance from the electrical power lines.
