OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to amputations and caught in hazards while using a conveyor: (a) On or about January 23, 2015, Mountain Man Nut & Fruit Company, did not ensure employees were protected from amputations and caught in hazards while clearing a jam in the conveyor of a high speed form and fill packaging machine while it was running. Abatement Note: Abatement certification and documentation are required for this item (see enclosed "Certification of Corrective Action Worksheet").

29 CFR 1910.219(c)(4)(i): Unguarded projecting shaft end(s) did not present a smooth edge and end and projected more than one half the diameter of the shaft: (a) On or about January 23, 2015, Mountain Man Fruit and Nut Company did not ensure employees were protected from machine hazards. The shaft ends projecting from the drive mechanism for the conveyor belts were not guarded. Abatement Note: Abatement certification and documentation are required for this item (see enclosed "Certification of Corrective Action Worksheet").
