1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. On or before 01/22/2015, the employer did not ensure that various equipment or machinery used at the site was effectively guarded. The unguarded equipment includes: a. Sixty-three (63) pneumatic clamps, located throughout the production area, did not have guarding; b. The rotating parts (e.g.: front jaw slot, chuck valve, etc.) on sixty (60) automated twisting machines that are actuated by a foot pedal was unguarded.

29 CFR 1910.212(a)(3)(ii): The point (s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): On or before 01/22/2015, point of operation guarding was not provided for the hydraulic shears used by operators tasked with cutting fill (material) in the production of wire brushes. Actuated by a toggle switch (that is similar to a household light switch), the employer failed to ensure that operators who utilized the presses were protected against having any part of their body in the danger zone(s) during operating cycle(s).
