29 CFR 1910.145(f)(4)(ii): A signal word was not readable at a minimum distance of five feet (1.52 m) or such greater distance as warranted by the hazard: On or about January 26, 2015, at the Advanced Facer Canceller System 200 (AFCS 200) machine #3, the emergency stop "STOP" label above the right hand emergency stop button was torn away, leaving the label illegible.
