29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: On or about January 23, 2015, at Toga Mfg, Inc on the production floor: Employees were exposed to amputation hazards when the employer did not develop specific lockout/tagout procedures for each machine and allowed employees to place their fingers in-between the machine guard of the Hydraulic Forming press without utilizing lockout/tagout procedures.
