29 CFR 1926.1053(a)(2): Ladder rungs, cleats, and steps shall be parallel, level, and uniformly spaced when the ladder is in position for use. (a) Raytheon Far Field Facility - On or about January 16, 2015, an employee fell from a ladder placed on top of an A/C Handing Unit. This employee was seriously injured and hospitalized due to a step ladder not being parallel and level.
