29 CFR 1904.29(b)(6): The employer did not enter "privacy case" in the space normally used for the employee's name on the OSHA 300 form or equivalent.   At the Sharon Lane Health Services facility on 10315 Johnson Drive in Shawnee Kansas: On and before 1/27/2015,  the employer did not conceal an employee's identity on the 2014 log when the employee suffered a medically private injury from a contaminated needle-stick. The employer did not report the needle-stick injury as a privacy case.

At the Sharon Lane Health Services facility on 10315 Johnson Drive in Shawnee Kansas: On and before 1/27/2015, the employer did not record an entry on the log when an employee suffered a contaminated needle-stick.
