The employer shall establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative.    When employees were required to perform servicing and maintenance the employer did not establish a program for the control of hazardous energy (lockout/tagout) for training and periodic inspection for those related undocumented procedures. This exposed employees to hazards associated with unexpected start up.    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1910.19(d).

29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc.    The employer failed to provide methods of machine guarding that would protect employees while operating the following equipment:    1.) Cake Slicer, UBE- The blades were not guarded to prevent employees from accidental contact when the cake slicer was in operation. There are holes approximately 3 inches in diameter that could allow employees to gain access to the moving blades inside the machine. This exposed employees to amputation hazards.     2.) Winkler Divider- Model 307.W-  The dough rolling table was not adequately guarded to prevent employees from being struck by its circular movement. The indexing dough divider and its arbor were also not guarded, this created a pinch point. Hinged sections of guarding were not fastened in place to prevent entry. Employees were exposed to struck-by and pinch point hazards that were likely to cause amputation.     3.) Vertical Mixer- The front guard for the mixer was missing, exposing employees to the rotating parts when the machine was in operation. This exposed employees to caught in and struck-by hazards.    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
