29 CFR 1910.147(a)(2)(ii)(B): An employee is required to place any part of his or her body into an area on a machine or piece of equipment where work is actually performed upon the material being processed (point of operation) or where an associated danger zone exists during a machine operating cycle. (a) The employer failed to ensure that he interlock on the Delta Flo wrapping machine was properly installed and utilized to provide employees with alternative means for the point of operation when conducting minor servicing activities. This exposed employees to amputation hazards.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) The interlock key (on the Delta 3000 LD DX wrapper machine) had been removed and inserted into the interlock switch, and employees were allowed to operate the machine with the gate open. This exposed employees to amputation hazards from the sealer/knife and the chain and sprockets.
