29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Crossbraces were used as used as a means of access.    WORKSITE: The ladder jack scaffold, from/on which employees were working were not provided with appropriate means of access and employees were using the ladder component and climbing over the walking/working surface in order to access it.

Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure.    WORKSITE: The employees, performing roofing activities on a 12/12 pitch roof were not protected against the hazard of falling between 12-feet, 9inches and 13-feet, 6 inches to the ground respectively, by use of an appropriate fall protection system.

29 CFR 1926.1053(b)(5)(i): Non-self-supporting ladders shall be used at an angle such that the horizontal distance from the top support to the foot of the ladder is approximately one-quarter of the working length of the ladder (the distance along the ladder between the foot and the top support).    WORKSITE: The 16-foot aluminum straight ladder, used to access the ladder jack scaffold was setup at an angle where the horizontal distance from the top support to the foot of ladder exceeded one-quarter the working length of the ladder.
