29 CFR 1910.178(n)(6): Industrial truck drivers were not required to look in the direction of, or/and keep a clear view of the path of travel:  a)  COSTCO, 465 Route 70, Brick NJ - Middle freezer, aisle 314:  A forklift driver did not have a clear view of the path of travel while entering the middle freezer located near aisle #314.  An employee got run over by a Crown Type E - RC5500 Series forklift # 5, Serial Number 1A334180.  Occurred on or about 1/19/2015.
