29 CFR 1910.176(a): Where mechanical handling equipment was used, sufficient safe clearances were not allowed for aisles, at loading docks, through doorways or wherever turns or passage had to be made: (a) The employer did not ensure that the space between the moving truck and loading dock was maintained to ensure that employees were not caught between the truck and the loading dock. This exposed employees to crushing hazards.
