29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: a. On or about January 15, 2015, at the above addressed jobsite, employees were exposed to a fall hazard of approximately 9 feet - 6 inches while pressure washing a residential roof, without the use of conventional fall protection or without a warning line system and safety monitoring system.
