29 CFR 1910.272(j)(1): The employer did not develop and implement a written housekeeping program that established the frequency and the method(s) determined best to reduce accumulations of fugitive grain dust on ledges, floors, equipment, and other exposed surfaces:  Employees performing elevator operations in the grain elevator were exposed to fire and explosion hazards in that the housekeeping program did not specify the frequency of housekeeping methods to prevent the accumulations of combustible dust at the far end of the grain elevator tunnel.  Combustible dust was measured to be 1/2 of an inch deep.

29 CFR 1910.272(j)(2)(ii): The employer did not immediately remove any fugitive grain dust accumulations whenever the exceeded 1/8 inch (.32cm) at priority housekeeping areas, pursuant to the housekeeping program for grain elevators:    Employees performing grain elevator and feed mill operation tasks were exposed to fire and explosion hazards.  Dust accumulations were allowed to exceed depths greater than 1/8 inch within 35 feet of the bucket elevator legs for the following locations:  a)  Grain Elevator Boot Pit, dust measured 4 inches deep. b)  Grain Elevator Tunnel under the conveyor belt, dust measured 1 1/8 inch deep. c)  Feed Mill Boot Pit, dust measured 1 foot deep.
