29 CFR 1910.147(c)(1): The employer did not establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: a) On January 27, 2015 and at times prior thereto the employer relied on informal procedures for the control of potentially hazardous energy when employees performed scheduled and un-scheduled maintenance on machines and equipment through-out the facility. Informal training and procedures did not ensure that authorized employees were provided with comprehensive and consistent information about the identification and location of energy sources, process of installing, removing, transferring and testing the lockout / tag-out devices. Note) Certification of corrective action IS required for this item.
