29 CFR 1910.304(g)(5): The path to ground from circuits, equipment, and enclosures was not permanent, continuous, and effective: (a) Employee(s) operating portable stud welders that were on carts at the facility were exposed to electrical hazards due to no grounding pin on the plug end of the power cable. (b) Employee(s) operating the portable Master heater in the Hem saw department were exposed to electrical hazards due to no grounding pin on the extension cord plug end, being used to provide power to the portable equipment.
