29 CFR 1910.133(a)(3):  The employer did not ensure that each affected employee, who wore prescription lenses while engaged in operations that involved eye hazards, used protection that incorporated the prescription in its design, or wore eye protection over the prescription glasses or protective lenses with side shields:     On or before 01/28/2015, employees who wore prescription lenses while engaged in operations that involved eye hazards were not provided with adequate safety eye protection.

29 CFR 1910.147(c)(4)(ii)(B): Specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy:      The company's lockout program was deficient and did not include specific instructions for isolating, blocking, or securing hazardous energy. Employees are tasked with servicing and maintaining equipment such as; but not limited to, the injection mold machinery which utilizes multiple energy sources (e.g.: pneumatic, hydraulic, thermal and electrical).

1910.212(a)(1):  One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks.      On or before 01/28/2015, the employer did not ensure that various equipment or machinery used at the site was effectively guarded.  The unguarded equipment includes:     a.  The chuck of the Jet Equipment & Tool lathe, which was located in the tool room of the facility, was inadequately        guarded in that the guard did not fit as intended.  The over-size guard was laid awkwardly across the top of the unit        with a piece of rubber extending downward however; the chuck itself remained uncovered thus potentially exposing        the operator to hazards created by the rotating part.     b.   The rotating chuck of (2) automatic drill presses were not guarded.
