29 CFR 1910.23(a)(2): Ladderway floor opening(s) or platform(s) were not guarded by a standard railing with standard toeboard(s) on all exposed sides (except at the entrance to opening) with passage through the railing either provided with a swinging gate or so offset that a person could walk directly into the opening:  Employee(s) climbing a fixed ladder up to a ladder platform along the north wall of the mold / pulverizer area were exposed to a fall hazard of approximately 11 feet due to no swing gate being provided on the ladder platform.

29 CFR 1910.23(c)(1): Open sided floor(s) or platform(s) 4 feet or more above the adjacent floor or ground level were not guarded by standard railings (or the equivalent as specified in 29 CFR 1910.23(e)(3)(i) through (v)), on all open sides:    (a) Employee(s) working by or entering the compressor pit area in the maintenance department were exposed to a fall hazard of approximately 84 inches due to no railing being provided next to the compressor pit stairway.  The missing railing was approximately 46 inches in length.          (b) Employee(s) working on the separator platform along the north wall of the mold / pulverizer area were exposed to a fall hazard of approximately 11 feet due to no top rail being provided for the northeast corner section of the platform. The northeast corner section of the platform with the missing top rail was approximately 27 inches in length.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  (a) Employee(s) working / walking by the Magnum 500 Wiedemann machine in the Steel shop (Fab side) were exposed to struck by hazards due to no machine guarding being provided on the right side of the machine where the parts table moves either east to west or north to south, depending on the equipment programming.  The right side of the machine had no guarding in place to prevent workers from being inadvertently struck by the parts table.  (b) Employee(s) operating the hydraulic Scale Gaging press in the Steel shop (Fab side) were exposed to caught in / between hazards due to no machine guarding being provided for the press cylinder.  The Scale Gaging press cylinder moves up and down during operation, this creates a pinch point for the operators hand / fingers.

29 CFR 1910.304(g)(5): The path to ground from circuits, equipment, and enclosures was not permanent, continuous, and effective:   (a)  Employee(s) using a yellow extension cord to charge the JLG manlift model #E450AJ in the Mold area were exposed to electrical hazards due to no grounding pin on the plug end of the extension cord.   (b)  Employee(s) using a panel strip to power the computer and monitors for the Amada Vipros 368 King II machine in the JIT Fab department were exposed to electrical hazards due to no grounding pin on the power cord plug end of the panel strip.

Covers of outlet boxes having holes through which flexible cord pendants pass shall be provided with bushings designed for the purpose or shall have smooth, well-rounded surfaces on which the cords may bear.

(b) Circuit breakers were not labeled inside the low voltage electrical panel box on column 7 in Jit Fab.  (c) Circuit breakers were not labeled inside the 277 / 480 volt lighting electrical panel box on column 7 in Grid machine department.
