29 CFR 1910.179(n)(3)(iii): During hoisting care shall be taken that there is no sudden acceleration or deceleration of the moving load. WORKPLACE: The employer did not ensure through visual observation and/or specific instruction that the load being hoisted by an overhead bridge crane was not suddenly accelerated in any directions. On or about January 23, 2015, an employee, who was working in very close proximity of the load being lifted was crushed in between the load and a flatbed truck due to a sudden acceleration of the load while being handled, and the employee sustained a fatal injury from the incident.

29 CFR 1910.179(n)(3)(iii)(b): During hoisting care shall be taken that the load does not contact any obstructions. WORKPLACE: The employer did not ensure through visual observation and/or specific instructions that the load being lifted by an overhead bridge crane was not only lifted up clearly from, but also did not contact any surrounding obstructions while being moved in any directions. On or about January 23, 2015, an employee, who was working in very close proximity of the load being lifted was crushed in between the load and a flatbed truck due to the load was not only lifted up high enough, but also a sudden acceleration. The employee sustained a fatal injury from the incident.
