WORKPLACE:  The employer did not ensure through visual observation and/or specific instruction that the load being hoisted by an overhead bridge crane was not suddenly accelerated in any directions.    On or about January 23, 2015, an employee, who was working in very close proximity of the load being lifted was crushed in between the load and a flatbed truck due to a sudden acceleration of the load while being handled, and the employee sustained a fatal injury from the incident.

The employee sustained a fatal injury from the incident.
