29 CFR 1910.215(b)(9): Guard for abrasive wheel machine where the operator stands in front of the machine was not constructed so that the peripheral protecting member could be adjusted to the constantly decreasing diameter of the wheel: Maintenance Department, On or about January 28, 2015, employees were exposed to struck-by hazards when using the Jet pedestal bench grinder model #JBG-8A serial # 12040732, which had a tongue guard on the stone side, but was adjusted at 1-3/4 inch from the stone, while employees use the grinder for grinding metal parts.

29 CFR 1910.253(b)(4)(i): Oxygen cylinders were stored near highly combustible material, especially oil and grease; or near reserve stocks of carbide and acetylene or other fuel-gas cylinders, or near other substance likely to cause or accelerate fire: Southeast corner, outside Maintenance Shop, On or about January 28, 2015 employees were exposed to burns, when three oxygen cylinders were stored together with three acetylene cylinders in a cylinder storage cage and were not separated from the fuel cylinders, while employees replace used with full cylinders on welding and cutting carts.
