29 CFR 1910.333(a)(2): Where exposed live parts were not deenergized (i.e., for reasons of increased or additional hazards or infeasibility), work practices that were used were not suitable for the conditions under which the work was performed:  a.) American Refining Group, Inc. - On or about January 28, 2015 and at times prior thereto, safety-related work practices including, but not limited to, the use of appropriate electrically rated personal protective equipment, was not required and in use during installation of an Allen Bradley brand 480 Volt starter.  The 480V starter was being installed into the Crude Unit Motor Control Center which was not de-energized prior to installation of the motor starter.

American Refining Group, Inc. - On or about January 28, 2015, and at times prior thereto, electricians were not trained in and familiar with the proper use of electrical related personal protective equipment during installation of an Allen Bradley brand 480 Volt starter.  The 480V starter was being installed into the Crude Unit Motor Control Center which was not de-energized prior to installation of the motor starter.

American Refining Group, Inc. - On or about January 28, 2015, and at times prior thereto, qualified persons were not familiar with the proper use of electrically related personal protective equipment during installation of an Allen Bradley brand 480 Volt starter. American Refining Group, Inc. - On or about January 28, 2015, and at times prior thereto, electrical protective rubber insulating gloves were not inspected every 6 months.  Sets of gloves were identified that were last tested in 2009 and 2011.  b.) American Refining Group, Inc. - On or about January 28, 2015, and at times prior thereto, electrical protective rubber insulating blankets were not inspected every 12 months.  Blankets were identified that were last tested in 2008.

29 CFR 1910.335(a)(1): Employees working in areas where there were potential electrical hazards were not provided with, and/or did not use, electrical protective equipment that was appropriate for the specific parts of the body to be protected and for the work to be performed:      a.) American Refining Group, Inc. - On or about January 28, 2015, and at times prior thereto, employees were not using appropriate electrical rated personal protective equipment including, but not limited to, arc flash hoods, gloves, sleeves, and insulating blankets, during installation of an Allen Bradley brand 480 Volt starter.  The 480V starter was being installed into the Crude Unit Motor Control Center which was not de-energized prior to the installation of the motor starter.  The appropriate electrical rated personal protective equipment for work involving energized equipment as required by the Employers SAF-26 procedure Electrical Safe Work Practices, and as referenced in the National Fire Protection Association (NFPA) 70E Table 130.7(c)(15)(A)(a), was not in use when the 480V motor starter was installed.
