29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees from hazards created by in-going nip points and rotating parts:    a)      Colonna's Shipyard dba Steel America, and Inside Machine Shop: On or about 01/23/15, a polishing disk fastened to the work holding device of an industrial lathe was not provided with a guard to protect employees from ingoing nip points, as a result an employee's left pinky finger was amputated.      Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
