29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. a) Left Side, Front of Structure - Two workers were observed on the top level of the structure affixing plywood to the floor/ceiling joist exposed to falling 15 feet off the edge of the structure or through the joist openings to the dirt ground below, on or about 01/28/2015. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement. 29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: a) Left Side, Front of Structure - The portable extension ladder employees used to gain access to the outside top level, 15 feet above the dirt ground, extended approximately 24" past the access level on or about 01/28/2015.
