29 CFR 1926.451(g)(4)(i): Guardrail systems shall be installed along all open sides and ends of platforms. Guardrail systems shall be installed before the scaffold is released for use by employees other than erection/dismantling crews.   a. On the north side of the residence where employees were working from a pump jack scaffold, adequate guardrail systems were not correctly installed along all open sides and ends of platforms, thereby exposing employees to a 14 foot, 9 inch fall hazard.

29 CFR 1926.452(j)(2): Poles shall be secured to the structure by rigid triangular bracing or equivalent at the bottom, top, and other points as necessary. When the pump jack has to pass bracing already installed, an additional brace shall be installed approximately 4 feet (1.2 m) above the brace to be passed, and shall be left in place until the pump jack has been moved and the original brace reinstalled.  a. On the north side of the residence where employees were working from a pump jack scaffold, the bottom of the scaffold poles were not secured, thereby exposing employees to a 14 foot, 9 inch fall hazard.

29 CFR 1926.501(b)(13): "Residential construction." Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure. Exception: When the employer can demonstrate that it is infeasible or creates a greater hazard to use these systems, the employer shall develop and implement a fall protection plan which meets the requirements of paragraph (k) of 1926.502.  a. On the north side of the residence where an employee was standing on a ladder on the same angle as the roof to install siding on the second story of the house, the employee was not protected from falls, thereby exposing the employee to a 12 foot, 9 inch fall hazard.  Or, in the alternative,  29 CFR  1926.1053(b) Use. The following requirements apply to the use of all ladders, including job-made ladders, except as otherwise indicated:  29 CFR 1926.1053(b)(4) Ladders shall be used only for the purpose for which they were designed.  a. On the north side of the residence where an employee placed an aluminum extension ladder against the roof at the same angle and was standing on the top step of the ladder to install siding on the second story of the house, the employee was not protected from falls, thereby exposing the employee to a 12 foot, 9 inch fall hazard.

29 CFR 1926.1053(b)(5)(i): Non-self-supporting ladders shall be used at an angle such that the horizontal distance from the top support to the foot of the ladder is approximately one-quarter of the working length of the ladder (the distance along the ladder between the foot and the top support).  a. At the worksite where employees used a ladder to access the roof, the ladder was 11 feet out from the roof edge, a height of 12 feet, 9 inches, thereby exposing employees to a fall hazard.
