29 CFR 1910.269(a)(2)(viii): The employer did not ensure that each employee has demonstrated proficiency in the work practices involved before that employee was considered as having completed the training required by paragraph (a)(2) of this section: (a) On or about January 20, 2015, for an employee who suffered serious injury from contact with live electrical parts operating at up to 14.4 kv while replacing a neutral line on a power pole near the intersection of County Road 53 and 104th Avenue NW, outside of Watford City, North Dakota.
