29 CFR 1910.119(e)(3)(v): The process hazard analysis did not address the hazards related to facility siting:    Warehouse Roof:  Employees who performed maintenance and inspection of the piping and valve connection systems were exposed to fall hazards where personal fall arrest systems, guard rails systems, or safety net systems were not used.  The process hazard analysis conducted in 2011 did not identify fall hazards where the equipment was close to the edge of the roof.

29 CFR 1910.145(c)(2)(i): Caution signs were not used to warn against potential hazards or to caution against unsafe practices:   Yard: Caution signs or markings on the ground were not implemented to warn employees against struck by hazards from heavy vehicular traffic or to alert truck operators of pedestrians walking in the yard.  On 01/29/2015 an emergency siren sounded which required evacuation of employees from the warehouse.  The employees walked through the yard without any identified caution signs for pedestrian walkway.
